Program/Service Notes

Crosspoint Human Services utilizes The Hoopeston Multi-Agency as a satellite office to provide an array of services to the Northern Vermilion County community. It will be necessary for the client to schedule an appointment with a staff person who will: 1. Assess needs and help clarify the client's thoughts, feelings and what he/she wants to accomplish; 2. Provide assistance with immediate problems; 3. Discuss types of services needed to help him/her reach goals; and 4. Arrange for the client to receive the services he/she wants. The client will be assigned a service coordinator who will follow the client's services and be his/her advocate until the client no longer needs the services. The types of services a person may receive include: A. SERVICE COORDINATION AND COMMUNITY SUPPORT: If a person needs assistance and support in getting help from other people or organizations. B. CRISIS INTERVENTION: If a person needs immediate response to problems too severe to wait for scheduled treatment, a crisis worker will assess the situation with the client and will acquire the necessary treatment. A crisis worker can be reached 24 hours a day by calling (217)431-1784. C. COUNSELING/THERAPY: Talking with one or two staff members individually, with the spouse, with the family or in a group to better understand him/herself, other people, the present situation and develop possible alternatives. A session usually lasts 30 minutes to an hour. There is no limit on the number of sessions. D. PSYCHIATRIC SERVICES: If a person is in need of a medical evaluation and/or medications, the client can reach one of the medical staff. E. PSYCHOSOCIAL REHABILITATION: Improving the client's day to day living and social skills, intellectual development, and psychological fulfillment for the adult experiencing psychiatric disabilities. F. DAILY LIVING SKILLS TRAINING: Improving the client's day to day living and social skills through individual and classroom instructions, recreation, and community activities, for the adult experiencing a significant developmental disability. G. INFANT DEVELOPMENT: for infants 36 months or younger, services are designed to enhance gross and fine motor body movement, speech and hearing, self-care, intellectual development, thinking and understanding and the capacity for social and recreational involvement. Infant services are provided at Crosspoint or in the home. H. HOUSING: If a person has trouble living independently, he/she can receive services while staying at one of Crosspoint's residential facilities. I. TRANSPORTATION: Is provided for Crosspoint clients only. Fees are variable dependent upon income and size of family. If the client has health insurance which provides coverage for Crosspoint services, the agency will file a claim for the full fee (actual cost) with the insurance company. The subsidies received from the government and the United Way will enable the agency to make substantial deductions for costs not paid through insurance coverage. The deductions are made through a sliding fee schedule. The sliding fee schedule is based upon the size of the family and the amount of family income. No one is denied service for inability to pay. Payment at the time of services is expected. Charges are made for uncancelled appointments.
