Program/Service Notes
VISION USA started in 1991 and is a program developed by doctors of optometry who are members of the American Optometric Association. To receive VISION USA services, individuals must meet ALL OF THE FOLLOWING CRITERIA: 1. Be working or be part of a household with one member working at least part-time or have lost their job within the past 90 days. 2. The applicant CANNOT have insurance that covers the cost of eye exams. (This includes Medicare and Medicaid). 3. The household income must fall below established levels based on household size. 4. Not have had an eye exam within the last 24 months. If an applicant (or family) is eligible, AND if there is a participating doctor available in their area, a letter will be sent giving them the name, address and phone number of he volunteer doctor who will provide a comprehensive eye exam at no charge. In some states Basic eyewear MAY also be provided at no cost or for a small fee/donation. Applications can be downloaded from the VISION USA website or call 800-766-4466 to apply by phone.
